Overview

The Frontline Honors takes place between October 1, 2024 and December 31, 2024. The Frontline Honors starts with a nomination process in which individuals or organizations will nominate a frontline worker for consideration to be part of the program. WTWH Healthcare editors will evaluate each entry in its respective category to determine the Frontline Honors class.

Eligibility

Nominees must be directly employed by a non-profit, for-profit, or governmental agency in the industries WTWH Healthcare and its publications covers and/or the organizations that support them. We will accept a maximum of two (2) nominations for individuals from any organization. Once the limit is achieved, any subsequent nominations received will be notified that the maximum number of submissions has already been reached for the annual program.

Each award entry fee must be paid prior to the entry deadline in order to be considered for eligibility for consideration and judging. Entry fees are non-refundable. Please keep in mind: those that are nominated and win this year will not be eligible for nomination again.

Nomination Process

Individuals, a corporation or governmental entity may nominate someone for the Frontline Honors program via the online submission starting October 1, 2024 and complete the process prior to 11:59 PM Central Standard Time on December 31, 2024 for consideration. Upon submission, each individual will be entered into the Frontline Honors nominee process. The form shall include the nominee’s name, organization, title, email address and information on their work for recognition based upon the program criteria. Each nominee, if not self-nominated, accepts these terms and conditions upon accepting the nomination hereby agrees to the terms and conditions of the Frontline Honors. By entering the Frontline Honors, entrants and nominees agree to consent to WTWH Healthcare using the personal information for the purposes of evaluating, judging and promoting as part of agreement to participation in the program. Failure to claim or accept the nomination or meeting the eligibility criteria may result in disqualification or forfeiture of the participation in the Frontline Honors class.

Competition Details

The Frontline Honors application process requires all the following components and responses as part of the submission:

  • The nominee’s name, title, company, and contact information
  • The nominator’s name, title, company, and contact information
  • Confirmation and acceptance of the nominee, if not self-nominated
  • Current resume or CV (Word or PDF format)
  • One headshot with appropriate photo credit (PNG or JPG format) The headshot should be at least 300dpi and should be no less than 1000px in width.
  • Written Statement: Please provide a 500-word statement outlining the nominee’s qualifications for the program. (Word or PDF format)
  • Two (2) professional letters of recommendation (one (1) internal to your organization and one (1) external to your organization) describing why the candidate excels as a frontline worker based upon the competition’s criteria, with contact information for each advocate.

Criteria:

  • Mission & Advocacy: Describe the positive contributions made by the nominee to the corporate and industry advocacy/mission.
  • Ethics and Professionalism: Describe how the nominee has demonstrated exemplary ethics and professionalism to both the company and the industry.
  • Innovative Spirit: Describe the nominee’s ability to drive innovation in their organization, their initiative, and delivering exceptional care.

Entry Fees:

  • Early bird entries cost $450 USD through November 29, 2024
  • Entries submitted from November 30, 2024 to December 31, 2024 are $550 USD

Evaluation

All submissions will be evaluated by the WTWH Healthcare editorial staff. Selection of a candidate as a Frontline Honoree is at the sole discretion of the WTWH Healthcare editorial staff. The entries selected for the Frontline Honors class for each category will be announced online on January 31, 2025.

Cancelation or Termination of Program

WTWH Healthcare reserves the right to cancel, suspend and/or modify the Honors, or any part of it, if any fraud, technical failures, virus, human error, tampering or any other factor impairs the integrity or proper functioning of the Honors, as determined by WTWH Healthcare in its sole discretion. In such an event, WTWH Healthcare reserves the right to award the prizes from among the eligible entries or plays received up to the time of the impairment. WTWH Healthcare reserves the right, in its sole discretion, to disqualify any individual it finds to be tampering with the entry process or the operation of the or to be acting in violation of the Honors terms and conditions of this or any other promotion or disruptive manner. Any attempt by any person to deliberately undermine the legitimate operation of the Honors may be a violation of criminal and civil law, and, should such an attempt be made, WTWH Healthcare reserves the right to seek damages from any such person to the fullest extent permitted by law. WTWH Healthcare failure to enforce any term of these Official Rules shall not constitute a waiver of that provision.

Limitation of Liability

WTWH Healthcare is not responsible for interrupted or unavailable network server or other connections; for errors in communication protocols; failed telephone, computer or electronic transmissions; for jumbled, scrambled or misdirected entries or transmissions; for phone, electrical, network, technology or computer hardware or software or program malfunctions, failures or difficulties; or any other errors, omissions, interruptions, or deletions of any kind, whether human, mechanical or electronic; or for non-receipt of any entry regardless of the cause of it not being properly received. WTWH Healthcare is not responsible for unintelligible, late, lost or stolen entries; for incorrect or inaccurate entry information, whether caused by Internet users or by any of the equipment or programming associated with or utilized in the Honors; or for any technical, printing or human errors which may occur in the receipt, non-receipt or processing of any entries in the Honors. WTWH HEALTHCARE MAKES NO WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE. SOME JURISDICTIONS MAY NOT ALLOW LIMITATIONS OF EXCLUSION OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES OR EXCLUSION OF IMPLIED WARRANTIES. Privacy Policy

Any personally identifiable information collected during an applicant’s participation in the Honors will be collected by WTWH Healthcare and used by WTWH Healthcare in accordance with WTWH Healthcare’s Privacy Policy as such is stated at https://agingmedia.com/privacy-policy-2/ WTWH Healthcare will share the nominators’ name with the nominee as part of the Honors program.. Beyond the usage described in these terms and conditions, the information provided will be collected, stored and used in accordance with WTWH Healthcare’s privacy policy.

Publicity Rights

By submitting nomination, nominees (or persons entering on behalf of nominee) shall allow WTWH Healthcare and/or WTWH Healthcare designees the perpetual right to use his/her entry, name, biographical information, photos, videos and/or likeness, and statements for promotion, trade, commercial, advertising and publicity purposes, at any time or times, in all media now known or hereafter discovered, worldwide, including but not limited to on the World Wide Web and Internet, without notice, review or approval and without additional compensation except where prohibited by law. A signed affidavit of eligibility, publicity release and release of liability may be required.

Governing Law

Issues concerning the construction, validity, interpretation and enforceability of these Official Rules shall be governed by the laws of Illinois.

Release

By participating, each nominee releases and agrees to indemnify and hold harmless WTWH Healthcare & Others from and against any and all costs, claims, damages, or any other injury, whether due to negligence or otherwise, to person(s) or property (including, without limitation, death or violation of any personal rights, such as violation of right of publicity/privacy, libel, or slander), due in whole or in part, directly or indirectly, to participation in the Honors.

General

WTWH Healthcare reserves the sole right to amend or alter the terms and conditions of the Honors at any time. By entering the Honors, the nominee agrees to abide and be bound by these Official Rules and the decisions of WTWH Healthcare, which shall be final in all matters pertaining to this Honors program. All entries become the property of WTWH Healthcare and will not be returned. Void outside of the fifty (50) United States/District of Columbia and wherever prohibited or restricted by law. All federal, state and local laws and regulations apply.
